Before or on your first appointment, you will need:

• Service agreement- complete, sign and return
• Provide copy of Medical Summary/letter from a doctor (GP, specialist or hospital) that includes confirmation of diagnosis and medical history.

The initial OT assessment is usually 1.5 hrs to enable us to gather necessary information. You can have a support person present with you. If you are under 18yo, a parent/nominee needs to be present also. 

Subsequent sessions run for the standard duration of 1 hour.

Yes. When working with children, our approach is to work with both the child and the parents /guardian. There is a strong focus on parent / carer training so that changes can be incorporated into your and your child’s daily routine as smoothly as possible.

Depending on the situation, there may be cases where clients of high school age, may not require a parent in attendance.
